Shoot 1: Risk Assesment

Student filming at Level -2 of The Rosebowl car-park, Leeds LS1 3HB – underground car park sub level 2 –  activity due 7-9pm 4th December 2013. Method statement attached. Site review/familiarisation visit took place – 29.11.13 – 3.30pm  - C.Domeracki – Senior H&S Adviser/Stuart Brown in attendance.

Hazards
Who might be harmed and how?
Control measures

LeedsMet security to be informed prior to activity starting – contact on the day by SB. Security contact number for city campus is 1003-8123154. Or via the emergency number 0113 812 4444.
Full briefing to all cast/crew at the start of filming activity by SB.

SB to record full crew/actors list + to  brief crew/actors to ensure appropriate behaviour for filming activity.

SB is the activity manager & to take charge/control as required.
Manual handling
Cast and crew, members of the public , LeedsMet staff
Equipment is mainly digital  recording - camera’s and audio.
Boom microphone/tripods and camera track will also be used.

Equipment will be shared via film crew and be within their lifting capability. Lift available for movement of equipment from street level to sub level 2 of the car park.
SB to ensure no-one is carrying too much. Take due care of any others persons present in the lift/car par areas.
SB to oversee movement of items.
Slips/trips/falls
Actors rolling down the tarmac.
Actors will be in leather costumes to give some protection. Plasters will be provided on set.
Rehearse the fall softly.
Vehicles
Cast and crew.
Area will be taped/cornered off. A team member will be responsible for looking out for the rare chance of approaching vehicles. All film crew members to wear high visibility vests during filming.
Ensure vests worn + watcher in place during activity.
Struck by moving object
Crew member trying to catch it.
Being hit with a plastic hammer A bed-sheet will be held up to catch by two crew members to catch  the hammer,  rather than a person trying to catch it with their hands.
Rehearse catching it in a sheet. Ensure no members of the public/ vehicles in close proximity
Trips and falls over tracking equipment and wires.
Any crew member falling.
Notify members when it is assembled, disassemble immediately after use. Tape loose wires down.
SB to check camera track/cables once set up is in place
Falling objects
Cast and crew.
Boom Mike will only be held when filming is happening to minimise tiring arms. If arms are tiring another team member will take over.
Crew backup in place
Equipment going missing .
College resources .
Member of crew – as designated watcher.
Every member of the crew and cast (when not filming) will be wearing high visibility vests, for safety amongst vehicles and to ensure people on set are part of my team
Vigilance by all crew + final review upon completion of activity
Damage to public property.
Public inconvenience and cost.
Parked cars will be highly avoided, if even present. Stunts will be performed away from cars, and filming will be stopped if pedestrians are remotely present.
SB to check on day & have final say.
Accidents/incidents
Film crew/LeedsMet staff, members of the public
Filming is taking place at quieter time.
Any accident/incidents must be reported to LeedsMet security – they are based at the front of the Leslie Silver Building – please note your mobile phones will not work in the underground - sub level 2.

You will need to go the street level – security contact number for city campus is 0113-8123154. Or via the emergency number 0113 812 4444.  First aid is available via LeedsMet security
Report as required.

Fire safety


Cast and crew.
 No fire drill tests are due on the evening. SB informed of smoke detection/fire alarm call points & fire exits available from this sub level of which there are 2 exits signed with green/white running man signage.
Fire alrm callpoints only to be used in an emergency.
Fire assembly point is surface car park – tarmac area adjacent to wooden fence – Woodhouse Lane side.

 Fire action notices are in place adjacent to fire exits – security contact number for city campus is 1003-8123154. Or via the emergency number 0113 812 4444
Vigilance on the day & action as  stated in control measures/fire action notices
